I Turhan Anik give notice that I have applied to Cornwall Council as Licensing Authority for the variation of a Premises Licence in respect of The Chicken Lounge, 6 Market Strand, Falmouth, TR11 3DB.

It is an offence knowingly or recklessly to make a false statement in connection with this application, the maximum fine on summary conviction being £5000.
