Representations must be in writing and may not be submitted by fax or email. A representation form is available from the Council website at www.elmbridge.gov.uk or from the Environmental Health & Licensing Division licensing@elmbridge.gov.uk It is an offence, knowingly or recklessly to make a false statement in connection with an application and the maximum fine for which a person is liable on summary conviction is £5000.
