Resident of Samora, Carrwood, Altrincham do hereby solemnly affirm and declare that in all my relevant documents my name and surname is recorded as Parwati Rai. That I used to sign my name as Parwati Rai for all intends and purposes. This is true to my knowledge. I sign this affidavit before the Notary at Hale, Altrincham on 20th April, 2018.
