JOINT PUBLIC CONSULTATION PROPOSED NEW PHARMACY
NHS Fife is in consultation with Dears Pharmacy regarding their proposed application for a new pharmacy to open within Unit 2, Duloch Park, Dunlin Drive, Dunfermline, KY11 8EX.
Dears Pharmacy propose to provide the full range of NHS pharmaceutical services, including any additional services required under an NHS arrangement.
The Proposed Opening Hours of the Pharmacy are:
Monday: 8.30-6.30 Tuesday: 8.30-6.30 Wednesday: 8.30-6.30 Thursday: 8.30-6.30
Friday: 8.30-6.30 Saturday: 9.00-5.00 Sunday: 10.00-4.00
Members of the public are invited to participate in the consultation. To take part please complete the Pharmacy Consultation Survey at www.nhsfife.org/pharmacyconsultation or alternatively a hard copy of the questionnaire can be obtained by contacting the Primary Care Department on 01592 226413.
As required under the Regulations this Public Consultation will be for a continuous period of 90 working days from the date of this Advert and will end at 5.00pm on 13 June 2018. No responses can be accepted after this date.
