Application forms can be collected from the Town Council Offices, Church Street, Ledbury, Herefordshire, HR8 1DH or can be requested by email at: reception@ledburytowncouncil.gov.uk The application form and Donations/Grants Policy can also be accessed via Ledbury Town Council's website: www.ledburytowncouncil.gov.uk

Completed forms should be returned to the Town Council Offices no later than 9th February, 2017.
