I, Mohamed Tamim Ansari Mohamed Kasim applying for a variation to premises licence at the premises 33-35 Estcourt Road, Salisbury, Wiltshire SP1 3AS and made to Wiltshire Council, Public Protection, County Hall, Bythesea Road, Trowbridge BA14 8JN.
I am applying for the following licensable activity premises licence for consumption off the premises such as Sale of Alcohol Mon - Sun 07.00 am 3.00 am for consumption off the premises.

It is an offence knowingly or recklessly to make a false statement in connection with an application. A person guilty of an offence under this section is liable on summary conviction to a fine not exceeding £5,000.
